Zephyrhills Community Based Outpatient Clinic

The Zephyrhills Community Based Outpatient Clinic is located at 6937 Medical View Lane, Zephyrhills, Florida 33542. The Zephyrhills Clinic Based Outpatient Clinic is just north of the intersection of Highway 54 and 30. Drive north on Highway 301. Turn right onto Daughtery Road (about 0.3. miles). The Zephyrhills Clinic Based Outpatient Clinic is adjacent to the Florida Hospital. Parking is free on premises. This CBOC was established to provide Primary care services for Veterans in east Pasco County.

Hours of Operation: Monday – Friday 7:00 am – 4:30 pm
Primary Care Services are not available on weekend or holidays.

Services Provided Include:

• Nursing Care: Nursing staff members include RNs and LPNs. The nursing staff is responsible for patient education, ordered procedures, immunizations, care management and other nursing tasks.
• Primary Care: Three Primary Care Providers, including one Women’s Health Provider. 
• Clinical Pharmacist: (Chronic Disease State Management and Anticoagulation Clinic)
• Health Administration Services (HAS)
• Laboratory Services: (Appointments Only: from 7am -11:45 am) 
• Mental Health: (full range of services)
• Nutrition (Nutrition Assessment and Coaching, MOVE! classes offered) 
• Social Work Services
• Telemedicine: Diabetes Management, MOVE Exercise, Smoking Cessation, Tele-Dermatology, Nutrition and Genetic Counseling.

Scheduling Appointments:

Don’t be a "No Show." Please call and reschedule clinic visits that you cannot make. This will ensure that another Veteran can be seen.

Emergencies:

The Zephyrhills CBOC is not equipped to treat medical emergencies. If you need urgent care, please call 911. Do not drive to the clinic or hospital. This will put you at risk!
