Lakeland Community Based Outpatient Clinic
The Lakeland Community Based Outpatient Clinic is located at 4237 South Pipkin Road, Lakeland, Florida 33811 in the Parkway Corporate Center near the Polk Parkway in Lakeland. This CBOC was established to provide comprehensive primary care services to Veterans who reside in Polk and surrounding counties.
Hours of Operation: Monday – Friday 8:00 am – 4:30 pm

PACT primary care appointments are scheduled one year in advance. If you wish to make, cancel or re-schedule a clinic visit, please call the Lakeland CBOC.
Don’t be a “No-Show.” Please call and re-schedule clinic visits that you cannot make. This will ensure another Veteran can be seen.
If you are feeling sick or feel that you need to be seen by your provider, contact your PACT team RN.
If you are admitted to a non-VA hospital, please contact your PACT team RN; they will assist in arranging a follow-up appointment if needed, secure your medical records and assist with other medical needs.
