A family doctor who's with you for life. Isn't that what we would all like? A new Patient-Centered approach implemented throughout VA hospitals in Florida and the Caribbean brings that concept closer than ever before in a simple, personalized health care experience designed to focus solely on the Veteran.

Every Veteran has a personal Primary Care physician who leads an interdisciplinary medical team called a PACT (Patient Aligned Care Team) designed to provide continuous and coordinated care throughout a patient's lifetime. The PACT includes a Registered Nurse Care Manager, clinical and administrative staff, and health care specialists.

The PACT unit works closely with one another as they provide continuous, comprehensive care for the Veteran during the various stages of life, providing early detection screenings, acute care (short term medical treatment), chronic care and end of life (palliative) care. The team uses education, lifestyle coaching, and ongoing communication and consultation to deliver whole-person care focused on keeping patients healthy.

The Veteran is a key member of this partnership—respected and encouraged to take an active role in his/her own care. Likewise, the patient's electronic medical record is vital to the entire effort as it pulls information together from multiple sources. Every test, medication and visit is recorded and available to the treatment team throughout the Veteran's lifetime—any time, any place.
