Application - Pharmacy Residency Program

    All applications are to be completed online through PhORCAS at https://portal.phorcas.org/

PGY-1 Application Requirements:

1. Applicants must be US citizens.

2. Applicants must possess a Doctor of Pharmacy degree from an ACPE-accredited program prior to program start date

3. Applicants must participate in the American Society of Health-System Pharmacist Resident Matching Program

4. Acceptance and participation in interview process

5. Completed Application in PhorCas by listed deadline

     a. At least 3 letters of recommendation
     b. Letter of Intent
     c. Pharmacy School Transcripts
     d. CV

6  Academic Requirements for consideration:

     a. GPA > 3.0
